The city is very concerned about the safety of all of it's citizens and they will contact you in the event of an emergency. In order to be able to do this quickly and easily they need you to fill out and return an
"Emergency Contact Information Form"
This form is available in 3 formats above which you can dowload & save, then fill it out and return to the city by mail or email. The PDF file is the most common format used and is editable directly on your computer. Save it to your computer, Fill it out by typing in the spaces, save it, then attach to an email and send it to the City Secretary at the following email address:
